This section is for letters, reports and request for medical records for the patients use only
If your request is for a third party form to be completed, this will need to be submitted to the surgery along with evidence of your consent to share information.
Letters and reports are not part of our NHS service and therefore are done in our own time and will a incur a non-NHS charge. The charge depends on the time required to prepare the letter. For further details of NHS service charges, please check our Practice Policies or contact the surgery.
The turnaround time for a letter request is up to 21 days.

We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and amicably as possible.
To pursue a complaint, either contact the practice manager via email firstname.lastname@example.org or call and ask to speak to the practice manager, who will deal with your concerns appropriately.
